Jim,

chan_usbradio does have the ability stop squelch crashes when using 
hardwared COS/CTCSS signaling by applying an audio delay and chopping off 
a selected number of ms of trailing audio after COS/CTCSS drops.

Try these changes in usbradio.conf:

[usb]

carrierfrom=usb		; could be usbinvert
ctcssfrom=no
rxdemod=flat            ; input type from radio -- *must* be set to *flat*
rxsquelchdelay=150	; Number of milliseconds to "chop"
