I'd say you have the polarity of the COS and/or CTCSS backwards somewhere, and Asterisk is being told your radio has an open squelch which then blasts noise for the duration of the time-out timer.  Are you sure it behaves properly from then on?  Does opening the squelch reset the TOT and the process start all over again?  If not, maybe your COS/CTCSS detection isn't truly working.
