Strange..have you tried the URI in a different USB port?

Perhaps a different USB cable as well?

Try checking with a multimeter to see if there is continuity from the URI 25 pin connector shield to the USB connector shield (ground)

The Pi is power supply is now 5.3v at the Pi, and the error is still there. Audio very garbled, and the URI LED has a weird winking double flash. Only a reboot cures it.
