Perhaps you don't have an EEPROM installed. Most people dont.

Look in usbradio.conf. There should be a parameter "eprom=1". (or something similar) Change it to "eprom=0" -- Sent from my Android phone with K-9 Mail. Please excuse my brevity and tpying errors.
