What could cause the Error: RX Input Adjust Failed?

DONE tries=12, setting=1000, meas=58, sqnoise=13
ERROR: RX INPUT ADJUST FAILED.

No RF input to the radio.

I'm attempting to setup a substitute receiver for testing, but am
experiencing difficulty setting it up, and not wanting to make a switch to
simpleusb, thus making the minimum number of changes.

The substitute receiver (Yaesu FT-1500M) is detecting CTCSS, with USB COR
out, and I'm using the speaker output. (see below conf file)

Any thoughts?
