Tim, Buddy, and Jeremy,

This is also an issue with NVDA and JAWS on Windows and Chrome.

Tim, the present way the dialog behaves, a user of screen reading
technology cannot find the input fields to enter a username and
password. I'd have to dig into it a little more to ascertain
exactly why, but the modal doesn't get focus or appear in the
document tab order.
