[App_rpt-users] What could I have possibly done wrong? Help please

Guessing that is a HamVOIP version, you should check with those folks 
for specific help.
  And you might mention to them which version chip in your sound fob.
But you might also try a reboot without it and then fresh boot and setup 
to other header port (left<>right).
Can't hurt.
