Looks like sd card is hosed, it’s running for now until another reboot. Will have to reinstall it on another card and give it a go. FYI: it’s a 16 gig Pny card, the only card in our setup that isn’t a sandisk ultra. This will be second time the Pny card has had trouble. The other 13-14 machines all use sandisk ultra’s... After booting, you may need to enable write mode manually and put your files on manually.
But it sounds like the image is a bit messed up.
You might want to try downloading anew and try again on a different sd if you have it.
Perhaps also check that the pi is able to get the required (enough) current and that it's cpu isn't getting to hot,
especially during a install. Just things to check/do.

Almost like sd card is in readonly mode... anything written to sd card is flushed after a reboot

How you are doing that would be unknown. But i think that is what you need to look for.
It will not do that automatically on it's own as I know it to be. Some other script invoking that ?
