when I was the local switched network manager in the AT&T GNOC (mission control for the company in Bedminster NJ) back in '99-'02, the most dangerous animal was a contractor with a backhoe or piledriver.  Especially Texas DOT putting in armco barriers along the interstates.  Summer was always the worst...because the crews were out in full force...and it doesn't take much for a backhoe to pull up buried fiber conduit(s) or a pile driver to send a metal I beam through the bundle buried along the right of way.

It happens.  All it takes is a 2' misreading of a drawing or non-updated elevation to put the bundle in harm's way.
